A 75+ Health Assessment is a thorough check-up specifically designed for Patients aged 75 and older. It helps us understand your overall health and catch any potential issues early. This assessment covers everything from physical health to mental well-being and ensures that you’re getting the right care to stay healthy and active.
-
During the assessment, we will:
Review your medical history.
Check your blood pressure, heart rate and other vital signs.
Discuss your medications to ensure they’re still the best options for you.
Look at your mental health, including how you’re feeling emotionally.
Talk about your lifestyle, including diet and exercise, to see if any changes could benefit you.
-
The assessment usually takes about 45 to 60 minutes. We want to make sure we cover everything thoroughly, but we also respect your time. If you need a break or have any concerns during the appointment, we can adjust as needed.
